Testing FAQ

At the beginning of each semester, students are asked to meet with their instructors to discuss their testing accommodations for exams. Students may arrange to take exams in the classroom if they choose, but must work out the details regarding accommodations with the instructor. If students wish to take exams in the testing center, they must complete a Student Testing Request form with their instructors and return it to the OSD at least 3days prior to exams. An exam not taken at the scheduled time will be returned to the instructor. It will be the student's responsibility to make new arrangements with the instructor and OSD if there are changes to the time/day of the test.

Testing accommodations may include, but are not limited to:

  • Alternative format (large print, Braille, etc.)
  • Computerized audio visual testing or reader
  • Extended time for tests and quizzes (unlimited time is not a reasonable request)
  • Distraction-reduced environment (in testing center)
  • Separate room
  • Use of scribe
  • Word processor for essay or short answer exams

Grayslake Campus

  1. 3 days prior to an exam, the student will fill out the "Testing Accommodations Request Form". Extra copies of this form are located outside the OSD (L112).
  2. Student will bring " Testing Accommodations Request Form" to the instructor to fill in: testing instructions, amount of time for the class to complete the exam, and signature. The Instructor will keep the "yellow" copy, attach to exam, and bring to OSD (L112) prior to testing date. If a reader is required, faculty may send test electronically.
  3. Student will bring completed white copy of " Testing Accommodations Request Form" to OSD (L112).
  4. Student will keep "pink" copy to show to Testing Center on test day. Note: I.D. is needed to take the exam.
  5. Testing Center will: Time student: 1-1/2 or 2 times the amount the class is allowed. Record start time and end time at bottom of form.
  6. Instructor will pick up exam from OSD (L112) - signature is required OR OSD will deliver to instructor's Division Office. After hours, exam can be picked up from Testing Center.

Lakeshore Campus

  1. 3 days prior to an exam, the student will fill out the "Testing Accommodations Request Form". Extra copies of this form are located in the Lakeshore Testing Center.
  2. The student will bring " Testing Accommodations Request Form" to the instructor to fill in: testing instructions, amount of time for the class to complete the exam, and signature. The Instructor will keep the "yellow" copy, attach to exam, and bring to Testing Center prior to testing date.
  3. Student will bring completed white copy of " Testing Accommodations Request Form" to Lakeshore Testing Center. If accommodations are requested (in addition to extended time), the Grayslake OSD office will prepare audio, enlarged text, etc., and forward to Lakeshore Testing Center.
  4. Student will keep "pink" copy to show to Testing Center on test day Note: I.D. is needed to take the exam.
  5. Testing Center will: Time student: 1-1/2 or 2 times the amount the class is allowed Record start time and end time at bottom of form.
  6. Instructor will pick up exam from Testing Center - signature is required! Testing Center will forward "white" copy of the " Testing Accommodations Request Form" to the Office for Students with Disabilities in Grayslake after test completion.

Southlake Campus

  1. 3 days prior to an exam, the student will fill out the "Testing Accommodations Request Form". Extra copies of this form are located in the Learning Assistance Center (V212).
  2. Student will bring " Testing Accommodations Request Form" to the instructor to fill in: testing instructions, amount of time for the class to complete the exam, and signature. The Instructor will keep the "yellow" copy, attach to exam, and bring to V212 (Learning Assistance Center) prior to testing date.
  3. Student will bring completed white copy of "Testing Accommodations Request Form" to V212 (Learning Assistance Center). If accommodations are requested (in addition to extended time), the Grayslake OSD office will prepare audio, enlarged text, etc., and forward to Southlake Learning Assistance Center
  4. Student will keep "pink" copy to show to Learning Assistance Center on test day Note: I.D. is needed to take the exam.
  5. Learning Assistance Center will: Time student: 1-1/2 or 2 times the amount the class is allowed Record start time and end time at bottom of form. Learning Assistance Center will forward "white" copy of the "Test Request Form" to the Office for Students with Disabilities in Grayslake after test completion.
  6. Instructor will pick up exam from Adm. Office - signature is required!
